Assessment packages start at $200.00. We are happy to walk you through what option is best for your needs and what's included in the package 

• medication prescription, monitoring, blood work, up to 3 follow-up appointments/month, medication refills, email communication for quick questions/concerns
• letters & forms
• referrals as needed

$125/month. We offer flexible payment options and walk you through what's included so you know exactly what to expect

• medication management, monitoring, and follow-up: fee per follow-up visit
• prescription renewals fee per refill request
• letters fee per letter
• Forms from third parties fee per page
